Do you Qualify?

This form should ONLY be completed by schools and not-for-profit school groups requesting one of the following forms of sponsorship/donation:

• Certificate for Centennial Center Park shelter reservation
• Certificate for Community Room reservation
• Certificate for event with the Mayor
• Monetary funding of $250 or less
